By completing this form you confirm that you have read the photography-filming policy - please say yes or no to each of the permissions below.
I give permission for my child to be photographed or filmed in accordance with the Drama Hub Photography and Filming Policy: as an aid in drama activities, to record and celebrate the work students are doing or as part of a Film making project – none of these photographs or films will be shared with anyone other than other students and their parents without my permission.
I give permission for photographs to be taken of my child and used within the club for display purposes.
I give permission for photographs to be taken of my child for use within other printed publications.
I give permission for photographs and videos to be taken of my child for use on the club’s website.
I give permission for photographs and videos to be taken of my child for use on the club’s social media pages.


Declarations
We sometimes offer the children a biscuit or chocolate at end of term parties and also offer a lolly or small sweet if children have had a recent birthday. Please state whether you agree to the following statement:

I have provided any dietary requirements in the "medical, allergy & dietary information" field above and I give permission for my child to be given a biscuit or sweet at Drama Hub, provided these requirements are adhered to.

In the event that my son/daughter is injured whilst at the Drama Hub Wendover I agree to my son/daughter receiving medication as instructed and any emergency dental, medical or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by the relevant medical authorities.
